A welcome to Stevenage South Primary Care Network (SSPCN). 

The Stevenage South Primary Care Network is made up of 4 core network practices who are commissioned to work together to provide a range of healthcare services to a population of 57,000 patients across Stevenage.  The core network practices include Bedwell Medical Centre (& Roebuck Surgery), King George (& Manor House) , Knebworth (& Marymead) and Shephall Health Centre.  The PCN is governed by a PCN board, with representatives from each practice, who provide oversight of the service and approval for strategy and future PCN plans and objectives

 

Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.  To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).  

We have a team of dedicated Link workers within our PCN, link workers work with patients to look at "what matters to me", time to explore issues and challenges and offer the opportunity to empower and encourage patients to take control of their own mental health and wellbeing, for more info and to self-refer to our Link worker team please click here

Citizens Advice Stevenage focuses on the advice needs of Stevenage residents. We provide free, independent and confidential advice and support. We aren’t driven by a one-size fits all approach, and provide support in person, over the phone and online. People also need different types of support at various times in their life.
